Terms and Conditions*

Idaho Physicians Network (IPN) is a statewide independent practice association of participating providers who work together to deliver a wide range of health care services. Your employer or insurance carrier has chosen IPN to assist you in obtaining quality, cost-effective medical services.

In an effort to provide you with the most current listing of IPN participating providers, a provider directory with search capabilities is available on the IPN website at www.ipnmd.com. It is important to know when you enroll with your employer or insurance carrier whether services are provided through an IPN participating provider although the continued participation of any one provider or hospital cannot be guaranteed. As with any directory, frequent changes occur. IPN is not responsible for inadvertent errors or omissions and no assurance can be given as to the complete accuracy.

It is your responsibility to verify that a provider is participating with IPN by calling your employer or insurance carrier or the IPN Provider Locator Service at 433-4610 (1-888-789-2346).

Providers Changing Status
This directory is subject to change as new participating providers may join or currently listed providers may no longer participate. Therefore, frequent verification is necessary to confirm IPN participating provider status before services are rendered.
Eligibility, Benefits, Precertification and Referrals
Should you have questions regarding eligibility, verification of benefits, precertification, referrals and specific covered services, it will be necessary for you to contact your employer or insurance carrier.
Covered Providers and Services
This directory does not guarantee the services of IPN participating providers will be covered under your health care plan. Be sure to consult your health plan benefit design description to determine what types of providers and services are covered. Failure to confirm services could result in a reduction in your benefits or increased "out-of-pocket" expenses.
Customized Benefit Design
Some employers have customized benefits, which may include or exclude some of the IPN participating providers listed. Please check your health plan benefit design for specifics.
Providers Practicing in a Group
Providers listed in this directory may practice in more than one group. Not all groups or all providers within a group are necessarily IPN participating providers. Please verify contracting status prior to making an appointment by calling the IPN Provider Locator Service at 433-4610 (1-888-789-2346) or by contacting your employer or insurance carrier.
Provider Location/Place of Service
Individual providers may practice in a variety of different office locations. The listing of a specific provider does not always mean that the participating provider status applies to all practice locations. Before you visit a provider, please verify that your choice is, in fact, an IPN participating provider at the location where you will be receiving services. You may also verify the provider by calling the IPN Provider Locator Service at 433-4610 (1-888-789-2346), or by contacting your employer or your insurance carrier.
Credentialed/Board Certified
IPN participating providers are credentialed pursuant to Utilization Review Accreditation Commission (URAC) guidelines. Providers verified as board certified with the American Board of Medical Specialties (ABMS), American Osteopathic Association (AOA), American Board of Podiatric Surgery (ABPS) or other such boards as may be applicable, are indicated by a designated symbol. Certain credentialing criteria is not applicable to selected providers/facilities and a symbol denoting providers as non-certified by IPN will be used to identify those providers/facilities.

How to Search

To Find a Provider or Facility:

Select between Providers, Facilities or All Providers and Facilities by clicking one of the buttons in section 1.
Narrow the search with any combination of selections and/or by typing in the Name field in the Narrow by (optional), section 2.
Click “Search Directory” in section 3 to launch the search.
Click the box for I agree to the Terms and Conditions on the first search.
Use Filter Results to modify the search results. Click More Details or Additional Locations for more information.
Click Start New Search any time to begin a new search.
Check the search criteria choices and retry if No Results Found appears.

About Search Help:
This Search Help section provides basic information about features related to the Find a Provider search. Items in bold type are arranged alphabetically for additional information. You may find CONTACT information for IPN at the top right corner of this site.

Accepting New Patients / Not Accepting New Patients:
Accepting New Patients is self-reported by the provider. Contact the provider to confirm whether they are Accepting New Patients as this is subject to change by the provider.

Accreditation:
Accreditation is a quality assurance standard which certain types of Facilities are required to maintain to ensure patient safety. This information is provided by the facility and periodically verified. Accredited Facilities display the abbreviation or name of the standard it complies to. Additional information about a specific Accreditation may be found by searching the internet or contacting the facility.

Add to list:
Click Add to list to temporarily add search results to My Provider List.

Additional Locations:
Some Providers or services may be available at more than one Location. Click Additional Locations in the search results to view these locations. This option will not appear if a provider lists only one Location.

All Providers and Facilities:
Selecting All Providers and Facilities in section 1 will return all provider types in the directory subject to any applied filters in Narrow by: (optional) or Filter Results.

All Specialties
Selecting All Specialties in the optional Narrow by: (optional) Specialty section on the Find a Provider or Facility home page will query results from all possible Specialties currently listed in the data. See Specialty for additional information.

Board Certification:
For the purposes of this website, Board Certification applies to medical physicians only. Board Certification indicates completion and recognition of additional training, testing and peer evaluations in a given specialty. Board Certification is self-reported by the provider and verified as part of the credentialing process. Providers may practice more than one Specialty and may or may not be Board Certified for each Specialty. Providers may be Board Certified in other specialties which they may choose not to list as a practice specialty. Contact the provider or search the internet to learn more about Board Certification.

City:
Search for any Provider by Zip Code or a combination of City, State and/or County under the Narrow by: (optional) Location during an initial search. Cities are included only for active provider Locations. Click Start New Search to change the search criteria.

Clear and Clear All:
Modify search results by removing some or all of the filters in Filter Results. Select Clear to remove a specific search filter result or Clear All to remove all the applied filters.

CONTACT:
Find CONTACT information for IPN by clicking on the CONTACT link at the top-right corner of this website.

County:
Search for any Provider or Facility by Zip Code or a combination of City, State and/or County in the Narrow by: (optional) Location during an initial search. Counties are included only for active provider Locations. Click Start New Search to change the search criteria.

Directory Last Updated:
Find a Provider or Facility data is updated weekly but is subject to schedule changes as needed. The date of the last update is located in the top-right corner of Find a Provider or Facility.

Click Email to send provider search results for contact and location information to an email account.

Facilities:
For the purposes of this web search, Facilities include all provider types such as “brick-and-mortar” and entity services which are not individual Providers. Some examples of Facilities and entity services include Hospitals, Durable Medical Supplies, Skilled Nursing, Ambulatory Surgery Centers, Hospice, Home Health, and may include other facility-based services often provided by individual Providers such as Physical Therapy, Speech Therapy and Occupational Therapy. Contact the providing facility or entity for specific details and types of services provided.

Filter Results:
The Filter Results may vary depending on search criteria selected and the results returned. Additional filtering may be accomplished by modifying the filter options or using Clear or Clear All.

Find a Provider:
The Find a Provider tab is a quick link to the Find a Provider or Facility home search page.

Gender:
Gender may be used as search criteria for either Physicians or Non-Physician Providers. Gender information for each Provider may be found in the search results under More Details.

Hospital Affiliations:
Hospital Affiliations indicates where a Provider has permissions or privileges to admit or treat a patient in a specific hospital. Permission levels or privileges may vary or have limitations. This information is self-reported by the Provider. Facilities listed are limited to in-network locations only. Click a Hospital Affiliation for information regarding a facility.

I agree to the Terms and Conditions:
Clicking the box for I agree to the Terms and Conditions indicates agreement to the Terms and Conditions. Agreeing to the Terms and Conditions is required to obtain search results. This requirement appears only on the first search for each browser session but applies to the entire search process. The Terms and Conditions may be read in their entirety before proceeding to the results page or any time by clicking on the Terms and Condition link at the bottom of the website.

Language:
Language information is self-reported by the provider and does not necessarily include the provider’s staff or others at a Location, nor does it indicate the level of proficiency in that Language. Choose from the Language choices in the Narrow by: (optional) Language section on the home page of Find a Provider or Facility.

Location:
Select either Zip Code or a combination of City, State and/or County in the Narrow by: (optional) Location section on the Find a Provider or Facility home page. Location results include the provider or facility name, address, phone number, a Map link and other details which may vary depending on the Provider Category. Location results for clinics or groups will list participating providers by address location. Providers, Facilities or other services not listed at a given address may not be in-network at a specific Location and in-network provider participation should be verified by contacting the provider or the patient’s benefit administrator before seeking and accepting services there.

Location Distance:
The Location Distance search filter is found on Filter Results and calculates distances based on the City or Zip Code selected in the Narrow by:(optional) Location section on the Find a Provider or Facility home page.

Map:
Click “Map” to view an internet map showing provider locations and to obtain driving directions.

More Details:
Clicking More Details will expand the view and show additional information based on provider types. Examples include Languages, Gender, Board Certification, Hospital Affiliations and Accreditation.

My Provider List:
Save search results and create a temporary list by clicking My Provider List.

Name:
Providers and Facilities, including clinics or groups, may be searched for by typing a full or partial name into the Name field in section 2, Narrow by: (optional) Name. Individual Providers should be entered by “Last name, First name”. Partial Name searches return any Name containing the exact combination of characters entered. An active display will show Name matches while typing and the selection will continue to reduce Name choices as characters are added. Select a displayed Name by clicking on the full name when it appears in the list.

Narrow by: (optional):
Reduce the number of search results in the initial search by choosing any combination of options in section 2, Narrow by: (optional), on the Find a Provider or Facility home page or by using Filter Results on the results page. If a search attempt returns No results found, check the selections made or widen the search by eliminating or changing filter choices.

Non-Physicians
Non-Physicians include any individual Providers who do not hold a title of MD or DO. Examples of Non-Physicians include Optometry, Physical Therapy, Social Worker, Counselor, Psychology, Nurse Practitioners, Physician Assistants and Acupuncturist.

No Results Found:
A No Results Found message indicates the provider is not listed in the Find a Provider or Facility directory by the criteria used to search. Check the spelling or filters to ensure the correct choices were made. Some providers may not be listed in the directory by choice or due to their provider or service type. Verify with the provider and the patient’s benefits administrator prior to seeking and accepting services for any provider not found on this website. Physicians
Physicians are individual Providers of medical service with titles of MD or DO.

Printer Friendly:
Clicking Printer Friendly generates a formatted version compatible with most printers and browsers to allow printing of the search results.

Providers:
For the purpose of the Find a Provider or Facility search, Providers are individuals while Facilities are non-individuals, such as Hospitals and other institutions or entity type services. Providers are separated into Physicians and Non-Physicians to facilitate searches by provider types and search result formats.

Provider Category:
Providers are grouped into the following four categories to facilitate searches based on similar provider types and different search result formats.

All Providers and Facilities includes all provider types for both Providers and Facilities. Some Specialties, for example, such as Physical Therapy, Speech Therapy and Occupational Therapy, may be provided by both individual Providers and Facilities and it may be helpful to locate services by using this option.
Physicians include only medical doctors having a MD or DO title.
Non-Physicians include all individual Providers who are not physicians.
Facilities & Hospitals include all provider and service types which are not individual Providers. See Facilities for additional information.

Providers in Group or Facility:
Clinics, groups, other practices or Facilities may be searched by their practice NAME. Participating Providers search results will be listed by participating LOCATION. Providers not listed at a given LOCATION may not be in-network at that LOCATION. Provider in-network participation should be confirmed by contacting the provider’s office or the patient’s benefit administrator before seeking and accepting service at a specific Location.

Reset Form:
Click Reset Form to clear selections made on the Find a Provider or Facility home page.

Save Results as PDF:
Click Save Results as PDF to convert search results to a PDF format to save to a location your web browser allows.

Search Directory:
Click Search Directory in section 3 to launch a provider search based on choices made on the Find a Provider or Facility home page.

Specialty:
Specialty is self-reported by the provider but verified during the credentialing process. A Physician described Specialty is limited to choices defined by the American Board of Medical Specialties (aka ABMS) where more information may be found on their website. Other listed specialties are commonly used terms within healthcare and standardized according to IPN policy and procedures. Some providers may limit or expand the areas of emphasis of their practice and it is recommended to contact the provider for additional or specific information regarding the scope of their practice and to always verify with the provider and the patient’s healthcare administrator whether a provider and specific services are covered by patient benefits before seeking and accepting services. Specialty names may change, be added or removed depending on available providers or changes in naming conventions.

Specialty Category:
The Specialty Category on the Filter Results is drawn from the list of specialties returned in the results based on the search criteria selected. Additional filtering may be achieved by choosing from the list within the Filter Results.

Start New Search:
Click Start New Search to clear results and return to the Find a Provider or Facility home page to begin a new search.

State:
Search for any provider by Zip Code or City, State and/or County. Select State from the drop down choices under Location in the Narrow by: (optional) section during the initial search. States are only included for active provider Locations. Use Start New Search to return to the home page to modify the search criteria.

Within:
Select from the dropdown options to define the driving distance from a given City or Zip Code to a provider Location. Exact will only include locations which are an exact match. Consider expanding the distance if No Results Found is returned.

Zip Code:
Search for any provider by Zip Code or City, State and/or County. Select Zip Code from the drop down choices under Location in the Narrow by: (optional) section 2 during the initial search. Zip Codes are only included only for active provider Locations. Use Start New Search to return to the home page to modify the search area.
